Gateway is a mental health service company, currently looking for experienced Mental Health Professionals to fill the position of Behavioral Support Professional, providing positive support for individuals experiencing interfering behaviors. The ideal candidate(s) must have obtained a MA/MS, or preferably a PhD/Psy.D in behavioral sciences or related field with demonstrated expertise in Positive (Behavior) Support services or therapy. The individual must meet the qualification of a Mental Health Professional as defined by the MN stat.S148B.50 to 148B.593. Additionally, Candidates with the following credentials are strongly encouraged to apply: LPCC, LMFT, LICSW, or Registered Nurse under Minn.Stat.S148.171 to S148.285. with certification as clinical specialist or nurse Practitioner in adult or family psychiatry and mental health nursing, or MA in nursing or related field, or its equivalent with at least 4000 hrs of Post-master’s supervised experience in the the delivery of clinical services. Hours: flexible
This position is available for full-time (40 hours) or part-time (25-35 hours). The Job Coach/ILS Specialist is able to create their own schedule and is responsible for contacting clients to meet their desired number of hours each week.
This position provides direct support services to individuals in their personal homes, who need assistance caring for themselves as a result of disability, mental health symptoms, and/or other inflictions.
This position is responsible for ensuring that service is delivered in a caring and respectful manner, in accordance with relevant Gateway Unlimited Living policies and industry standards.
Services May Include:
- Positive Support Specialist (PSS)
- Positive Support Analyst (PSA)
Required Job Responsibilities:
- Logging client hours worked on Google Sheets daily
- Checking company email each workday, remotely if necessary
- Keeping prompt communication and contact with clients and support teams via phone, email, and occasionally text (per employee discretion)
- Visit the Gateway Office in Burnsville at least once per week to physically log client notes and hours in respective client books
- Attend all Gateway staff trainings
- Self-manage client schedule ensuring all hours are being met each week and communicating to management with any issues
- Be on time to all scheduled visits and meetings
- Participate and write client 245D documents, support plans, etc.
- Maintaining strict confidentiality for all consumer information, and facility operations
- Completing all necessary documentation to assure compliance with all state and federal regulations. These may include, but are not limited to: daily data sheets and clinic notes, attendance logs, time records, incident reports, behavior reports, accident reports, seizure reports, etc.
- Complete an individualized functional behavior assessments
- Work with the person to develop a person-centered, positive-practice, individualized behavior support plan and/or positive support transition plan that identifies specific, proactive, and reactive intervention strategies, if necessary.
- Distribute the plans to the person and his/her team members
- Evaluate effectiveness of the service and interventions
- Modify the plans as necessary
- Provide onsite instruction learning to staff and caregivers about the use of positive supports
- Train and supervise waiver positive support analysts and waiver positive support specialists.
- Any tasks given / directed by a supervisor / management.
Required Qualifications & Education:
- Current valid driver’s license
- Proof of proper vehicle insurance
- Psychologist licensed under Minn. Stat. 148.88 to 148.98
- Independent clinical social worker licensed under Minn. Stat. Chapter 148D
- Master’s degree in social work from an accredited college with at least 4,000 hours of post-masters supervised experience in the delivery of clinical service in the competency areas
- Physician licensed under Minn. Stat. Chapter 147 and certified by the American Board of Psychiatry and Neurology or eligible for board certification
- Licensed professional clinical counselor under Minn. Stat 148B.50 to 148B.593 with at least 4,000 hours of post-masters supervision
- Master’s degree or Ph.D. in one of the behavioral sciences or related field.
- Registered Nurse licensed under Minn. Stat. 148.171 to 148.285
- MUST pass Background Study and DHS Fingerprinting
- Experience in areas of employment directly related to the performance of this job as included in the above “Job Duties and Responsibilities”

This position is a form of Targeted Case Management that provides coordination of activities to help a person who resides in an eligible institution gain access to medical, social, educational, financial, housing, and other services and supports that are necessary to move to the community.

Required Job Responsibilities:
- Receiving Referrals
- Outreach to case managers / care team members to ensure that the requested services are appropriate
- Expand and connect with care team to receive all appropriate release of information
- Support the person served and their chosen caregivers in developing a plan for long term stability
- Provide Person-Centered Planning offering a customized plan including housing, expungement, transportation, and community resources.
- Having knowledge of geographical areas and housing resources and tools to support a person’s plan for stability after discharge
- Coordination and collaboration with other Targeted Case Management services as well as managed care organizations
- Knowledge of local community resources and hot to use those resources for the benefit of the person
- Developing and reviewing a relocation plan with a person
- Making referrals to needed services and supports
- Communicating with the person to carry out the relocation plan in the 180-day period before discharge
- Coordinating overall service delivery to ensure quality of services, appropriateness and continued as needed
- Traveling
- Completing documentation necessary to develop and carry out the relocation plan
